What a stay includes.
A recuperative care stay is short and has a job to do: finish the recovery the hospital started, and leave with somewhere better to go than the street.
Medication Support
Reminders and coordination so prescriptions stay on schedule through the stay.
Care Coordination
A point of contact who keeps the hospital, the health plan and the follow-up provider working from the same information.
Appointment Scheduling
Follow-up visits booked and tracked so discharge instructions do not get lost in the move.
Transportation
Rides to and from medical appointments during the recuperative stay.
Behavioral Health Connection
Referral and coordination with mental health and substance use services when they are part of the recovery.
Benefits Assistance
Help applying for the benefits and identification documents that housing and ongoing care depend on.
Housing Navigation
Working toward a stable place to go next, rather than a return to the street at the end of the stay.
Transition Planning
A documented plan for what happens after the authorized stay ends, built from the day the stay begins.
These describe the recuperative care service model. Staffing levels, clinical coverage and bed availability differ by location. Call for specifics on a particular site.
Contracted services
Two Community Supports, under one contract.
Recuperative Care (RC) covers the medical respite stay itself: a bed, oversight, and coordination while someone recovers.
Short-Term Post-Hospitalization Housing (STPHH) covers housing immediately after a hospital or facility stay for members who need somewhere stable to continue recovering.
OSP Health Management Inc is contracted with Molina Healthcare of California for both.
