The process

Call us
Tell us the county, the health plan, and what the patient needs. We will tell you whether we can take them.

Send the paperwork by secure fax
A completed referral form, the history and physical, and the discharge summary. Send by secure fax. Please do not email patient information.

We confirm the bed
We confirm placement with you and coordinate the arrival time directly.
